SCL Program Process
Application / Qualifications
Families must complete an application and meet various guidelines to be considered.
- Be 7-18 years of age;
- Have “good” oral hygiene;
- Have a moderate to severe need for braces;
- Not be wearing braces currently;
- Family must meet the SCL financial guidelines (varies by geographic location); and
- Be willing to pay the non-refundable $30 (USD) application fee and the non-refundable $650 (USD) required financial investment (per child)
Screening Appointment
Once the information in the application is verified, families are sent to an intial consultation to see if they are a good fit for the practice.
Treatment Begins
Once the child has been approved for treatment in your office, we complete the paperwork and treatment begins.
We work with a number of suppliers that will donate materials including brackets, aligners, retainers, etc. so that you have no out of pockets costs.
Doctor's Choice / Fast Track Program
Ortho providers can also refer families who come through their office. since these are kids that have already been screened, they can be “fast tracked” to the head of the line.
The Doctor’s Choice program is designed to allow you (Smiles Change Lives provider) to select patients you want to treat through our ‘fast track’ application process so you can begin treatment inmediately. The Doctor’s Choice program also allows you to treat candidates that may not have qualified under the standard SCL guidelines (age, finances, serverity, etc.), but that you would like to treat.
Examples include:
military families
special needs patients
first responders (police officiers, fire fighters, etc.)
teachers’ families
patients that have fallen on hard times
older kids or students
*Simply email Mr. Alexis Barclay at alexis@smileschangelives.org
with the name and phone number of the candidate. We’ll conduct the screening process and refer them back to you as an official SCL patient for treatment.
